セブンスター・メンソール・12 launched on 14 April 2014, the same day as the Menthol 5, and it is the high-tar end of the brand’s menthol line. The blend is the domestic one, with menthol.
The 2014 date is a revival rather than a debut. Seven Stars menthol products go back to 2000, and the 12 mg figure has been used more than once — a セブンスター・ディープ・メンソール・ボックス sat at 12 mg and 0.9 mg from February 2011 until May 2014, and Wikipedia names this product as its successor. The menthol line has been rebuilt around the 12, 5 and 8 mg points ever since.

The nicotine figure is contested. JT’s current product table gives 1.0 mg; ja.wikipedia’s spec table gives 0.9 mg for the same product. Both are recorded here: JT is taken as authoritative for the product as sold today, and the archive does not pretend the disagreement is not there.
Here the disagreement has a plausible mechanism: the 2011 predecessor was 0.9 mg, so a reformulation when the product was relaunched in 2014 would explain JT’s higher current figure. That is an inference from the predecessor’s published spec, not something either source states, so it is recorded here as context rather than as an explanation the sources support.
